Community Health Clinics

Offered by Santa Clara Valley Medical Center, this program helps qualified patients to receive grant-funded financial assistance to offset high out-of-pocket expenses for life-saving medications such as insulin for diabetes, asthma inhalers, and/or EpiPens.
